HOME Choice

What is HOME Choice?

Ohio’s HOME Choice program assists a family with the transition of their child from a residential treatment facility back to their home and community. HOME Choice is used to help ensure a smooth and successful transition and to help prevent a return to a residential treatment facility. Families and youth who participate in HOME Choice have access to a HOME Choice Transition Coordination, HOME Choice Case Manager, transportation funds, goods and services funds, and a wide array of other HOME Choice approved services that help promote long term success at home.

Can my family receive support through HOME Choice?

Eligibility for a family and youth to participate in the Clermont County Family & Children First HOME Choice program includes:

  • Family must have a youth between the ages of birth to 21 who is currently placed in a qualified residential treatment facility
  • Youth must be in facility for at least 90 days prior to discharge date
  • Youth must be on Medicaid
